ROBOTIC SURGERY · HONEST GUIDE

Robotic vs Open Surgery An Honest Comparison

Dr. Nitin Shrivastava doesn't push robotic surgery for its own sake. He explains when robotic is clearly better, when open surgery is the right call, and what the evidence actually shows.

The Big Comparison

All approaches have a place - the right choice depends on the procedure, patient, and surgeon expertise

Factor Robotic Surgery ★ Laparoscopic Open Surgery
Incision size 4–5 ports (5–12 mm) 4–5 ports (5–12 mm) 15–30 cm incision
3D vision Yes - 10× magnified 2D (some 3D scopes available) Direct vision
Instrument dexterity 7 degrees of freedom + tremor filter 4 degrees of freedom Full - surgeon's hands
Blood loss Minimal (~100–200 mL) Low (~150–300 mL) Higher (~300–600 mL)
Transfusion rate <2% ~3–5% ~10–15%
Hospital stay 2–4 days 2–4 days 5–10 days
Return to work 2–3 weeks 2–4 weeks 4–8 weeks
Post-op pain Low Low–moderate Significant
Complex reconstruction Excellent (wristed instruments) Technically demanding Straightforward
Cost Higher (instrumentation) Moderate Lowest
Best for Prostatectomy, partial nephrectomy, pyeloplasty, cystectomy Nephrectomy, adrenalectomy, simpler urological procedures Very large tumours, trauma, redo surgery, complex adhesions

Best Approach by Procedure

Radical Prostatectomy

Preferred: Robotic

Urethrovesical anastomosis precision significantly impacts continence and potency recovery. Robotic gives clear advantage.

Partial Nephrectomy

Preferred: Robotic

Renorrhaphy with warm ischaemia <20 min is far more achievable robotically. Kidney preservation is superior.

Radical Nephrectomy

Preferred: Robotic / Lap

Both give equivalent results. Robotic preferred for large or complex tumours; laparoscopic for straightforward T2.

Pyeloplasty

Preferred: Robotic

The anastomosis requires precise suturing - robotic instruments outperform hand-sewn laparoscopic sutures.

Radical Cystectomy + Neobladder

Preferred: Robotic

Intracorporeal neobladder construction is now possible robotically - significantly less morbidity than open.

Large Renal Mass (>12 cm)

Preferred: Open

When tumour involves major vessels or requires vena cava thrombectomy, open surgery provides better control.

Dr. Nitin's Honest Take

"Robotic surgery is a tool - an exceptional one for complex reconstructive and oncological procedures. But the most important factor in any surgical outcome remains the surgeon's training, experience, and judgement - not the platform."

"I choose the approach that gives you the best outcome, not the most impressive one. Sometimes that's robotic. Sometimes it's laparoscopic. Occasionally it's open. I'll always explain my reasoning."
