ROBOTIC SURGERY · KIDNEY PRESERVATION
A kidney tumour doesn't always mean losing the whole kidney. Dr. Nitin Shrivastava uses robotic precision to remove only the tumour - protecting your kidney function for life.
For tumours ≤7 cm, partial nephrectomy is the global standard of care
| Factor | Partial Nephrectomy ✓ Preferred | Radical Nephrectomy |
|---|---|---|
| Kidney preservation | Yes - only tumour removed | No - entire kidney removed |
| Cancer-specific survival (T1) | Equivalent | Equivalent |
| Long-term CKD risk | Lower (more nephrons preserved) | Higher |
| Cardiovascular risk | Lower (kidney function maintained) | Higher |
| Surgical complexity | Higher - requires expert robotic skill | Lower |
| Hospital stay | 3–4 days | 2–3 days |
| Best for | T1a/T1b tumours; single kidney; bilateral tumours | T2+ tumours; complex hilar location |
3D CT reconstruction maps the tumour in relation to renal vessels and collecting system. Dr. Nitin uses this "roadmap" to plan the excision trajectory.
4–5 small incisions (5–12 mm). The Da Vinci robot is docked and 3D magnified view established.
Renal artery and vein identified and looped. Bulldog clamp applied at start of excision - the ischaemia clock begins.
Tumour cut out with a 2–3 mm margin using cold scissors under direct vision. Entry into collecting system is repaired with absorbable suture.
Kidney parenchyma closed in 2 layers with haemostatic agents. Clamp released - perfusion restored, ischaemia clock stopped.
Drain removed day 2–3 if output is clear. Most patients discharged day 3–4; outpatient review at 6 weeks.
