Robotic Partial Nephrectomy: The Procedure

Pre-op planning

3D CT reconstruction maps the tumour in relation to renal vessels and collecting system. Dr. Nitin uses this "roadmap" to plan the excision trajectory.

Robotic port placement

4–5 small incisions (5–12 mm). The Da Vinci robot is docked and 3D magnified view established.

Hilar control

Renal artery and vein identified and looped. Bulldog clamp applied at start of excision - the ischaemia clock begins.

Tumour excision

Tumour cut out with a 2–3 mm margin using cold scissors under direct vision. Entry into collecting system is repaired with absorbable suture.

Renorrhaphy

Kidney parenchyma closed in 2 layers with haemostatic agents. Clamp released - perfusion restored, ischaemia clock stopped.

3–4 day discharge

Drain removed day 2–3 if output is clear. Most patients discharged day 3–4; outpatient review at 6 weeks.
